NEW PATIENTS WELCOME!

Dr. Eric Christensen is currently welcoming new patients, both children and adults. With a focus on individualized patient care, our patient’s well-being is our priority and we want to transform first-time visitors into regular patients. Patient care begins before you even walk through the door – we want to make sure that we’re prepared to answer your questions over the phone and make appointments that accommodate your schedule.

We have made new patient forms available online for you to fill out prior to your Westhampton Dental visit. By clicking on the link below, you will be redirected to a secure health history form for you to fill out. Thank you!

OFFICE POLICY

It is our mission to provide you with affordable, high-quality treatment. Payment is due for services rendered at the time those services are provided unless you have made other arrangements with us in advance. We offer a variety of payment options to meet your needs. Our office accepts payments by cash, check, Visa, MasterCard, American Express, and Discover cards. If you have any questions about financing or payment, please ask us!

INSURANCE

If your insurance covers treatment, you will receive the benefit of reduced personal costs. Our goal is to help you achieve and maintain optimal dental care. Our office will do everything possible to help you make the most of your dental insurance benefits.

Please understand that dental insurance is a contract that is created between your employer and the dental insurance company. The benefits you receive are based upon the terms of the contract that were negotiated between your employer and the insurance company, not the dental office. Unfortunately, some of the services that you may need or want will not be covered by your dental insurance.

As a courtesy, we will file insurance claims for our patients to insurance companies in order for you to receive reimbursement. All patients are expected to pay their estimated portion on the day treatment is received. Since all insurance plans differ in coverage, deductible, Usual and Customary Rates (UCR), etc. we can only estimate what your insurance company will pay. If there is any balance or credit remaining after your insurance company pays, then you will be billed or credited the difference.

If you have any questions concerning our office financial policy, please call us at 631-288-4422 or email us. We will be happy to answer your questions.
